In consideration of being allowed to participate in any way in the activities and programs offered by Kinshasa Lifestyle, I, the undersigned, hereby acknowledge and agree to the following terms and conditions:

 

1. Assumption of Risk: I am aware that participating in fitness activities involves certain inherent risks, including but not limited to muscular injuries, sprains, strains, broken bones, heart-related injuries, and other potential risks. I acknowledge that I am voluntarily participating in these activities and assume all risks associated with them.

 

2. Health and Medical Condition: I hereby affirm that I am in good physical condition and do not have any known medical conditions that would prohibit my participation in fitness activities. I have consulted with a physician or medical professional and have been given clearance to participate in physical activities at Kinshasa Lifestyle Bootcamp.

 

3. Waiver of Liability: I, on behalf of myself, my heirs, and assigns, hereby waive and release any and all rights, claims, and actions that I may have against Kinshasa Lifestyle, its owners, employees, instructors, and agents for any injury, damage, or loss that may occur as a result of my participation in fitness activities at the facility.

 

4. Photo and Video Release: I grant Kinshasa Lifestyle the right to use any photographs, videos, or other media taken of me during my time at the facility for promotional and marketing purposes without compensation or further consent.

 

5. Personal Belongings: I understand and agree that Kinshasa Lifestyle is not responsible for any loss or damage to my personal belongings while on the premises.

 

6. Compliance with Rules: I agree to comply with all gym rules, regulations, and instructions provided by Kinshasa Lifestyle and its staff. Failure to comply with these rules may result in immediate termination of my membership or access to the facility.

 

I have read this waiver and fully understand its contents. I am aware that I am releasing certain legal rights that I or my heirs, next of kin, executors, administrators, and assigns may have against Kinshasa Lifestyle. By signing this document, I acknowledge that I am voluntarily agreeing to these terms and conditions without any inducement, assurance, or guarantee being made to me.
